You,
the patient, are our foremost concern. Without you, there is no practice.
We strive to treat each patient with the utmost respect, care, and concern,
and to provide the finest dental care available.
Our team approach to dental care involves you in the decision-making
process, from determining your needs to creating a customized dental
plan. We invite your questions and appreciate your suggestions at any
point along the way.
We believe
that a successful dental practice is comprised of more than just an
excellent staff and state-of-the-art equipment. Enduring relationships,
with open communication and mutual trust between the staff and the patient,
are also essential. Equally important are long-term results that are
not only aesthetically pleasing, but also comfortable and affordable.
To achieve these goals, we will work with you and your insurance company
to make certain that we are able to take full advantage of your benefits.
During the
course of your treatment, we will make every effort to listen carefully
to your concerns, needs, and desires. At the same time, we will work
to educate you, so that you will always be well-informed about the state
of your dental care, and so that you can make quality choices and decisions.
Developing and implementing an individualized treatment plan takes time,
effort, and collaboration. If necessary, we will enlist the services
of other specialists in order to complete your treatment. We also ask
that you participate in follow-up visits and maintenance care after
any restorative work is completed.
Your First Visit
We ask all new patients to fill out a Personal
Information Form and
a Medical and Dental Information Form. You
can save time by printing both forms, filling them out completely,
and bringing them to your first appointment.
From the start, we will work with you to develop an individualized
treatment plan, with the dual objective of reaching your expectations
and staying within your budget. We welcome any questions you might
have during your appointment and look forward to meeting you.
Continuing Care Visits
We believe in the benefits of preventive care, and we encourage
our patients to schedule periodic continuing care visits.
At the time
of these vists, please advise our business assistant of any changes
to the following:
1. health and medications
2. address or telephone numbers
3. marital status
4. employment and insurance coverage
Referrals
When
necessary, we refer dental problems that require specialized care to
the appropriate dental specialist.

children?
A: Yes.
Q: Can a minor
come to an appointment unaccompanied?
A: Yes, but only if he or she has written authorization for treatment
from a parent or legal guardian.
Q: Will I get
my teeth cleaned at the first visit?
A: Usually not, unless this is discussed ahead of time.
Q: Can I bring
x-rays from my former dentist?
A: We encourage you to. We may need current x-rays, but the old ones
are also helpful.
Patient Privacy
Our practice adheres to federal and state standards to maintain the
privacy of your health information. As required by law effective April
13, 2003, a complete notice of
your privacy rights as a patient is posted in our office. Please
